Explanation of Aphorism 104 Importance of keeping case record. After accurately sketching the picture of disease, most difficult part is accomplished. This case record is mostly important in case of chronic diseases, as -  1. Physician has the record before him so as to guide him in treatment.  2. Changes occurred after medication can be known by making fresh record and comparing it with former.  3. Any new symptom which has occurred during the treatment can be added to it, and that which has faded off is cut or stricken. In footnote Dr. Hahnemann says that old school physician of his time do not maintain record of the case rather make a rapid prescription composed of variety of ingredients whose true effect is unknown to them. They neither listen to minute detail of suffering of the patient nor write them down. Explanation of Aphorism 103 In chronic diseases Psora should be investigated.  In case of epidemic diseases homoeopathic medicine pertaining to it could be found by observing several patients suffering from same epidemic. Similar is the case with chronic miasm Psora, which is so diversified in its manifestation that it requires several patients of different constitution and living under different conditions suffering from Psora to be observed carefully to know the complete range of symptoms it can produce. Explanation of Aphorism 102 Complete signs and symptoms of an epidemic can be known by observation of several patients.  Writing down symptoms of several cases of an epidemic completes the totality impregnating it with more peculiar, characteristics and excluding general symptoms like anorexia, insomnia etc.  Epidemically effected persons are affected by same cause and present more or less similar symptoms, but whole totality of epidemic cannot be known by mere observation of single patient as he will be presenting only part of symptoms in intense degree pertaining to his weaker part. Finding homoeopathic medicine in epidemic diseases.  Initially by having a first case of an epidemic, physician may not obtain complete picture of the epidemic rather by close observation of several such cases he comes to know regarding its totality and arrives at suitable homoeopathic remedy of that epidemic.  Dr. Hahnemann says that by close observation of even one or two such patients in an epidemic, physician can make an idea regarding the totality of it at the back of his mind which nearly matches reality.  Once homoeopathic medicine of an epidemic is found, it can also be used as preventive of that epidemic.

Explanation of Aphorism 98 Demands to be met in investigation of disease.  Patient's description of disease is to be given special place except malingers, as the complaints and sensation gets altered in mouths of attendants and friends.  In every disease especially chronic, for investigation of true and complete picture of disease following are required -  1. Special circumspection,  2. Tact,  3. Knowledge of human nature,  4. Cautiousness in conducting enquiry,  5. Great patience.   Dr. Hahnemann says to have 'Knowledge of human nature' which will help in forming true and complete picture of the disease. This knowledge can be known by study of Anthropology, Physiognomy, Psychology Graphology, etc.

Aphorism 97: Indolent patients Indolent patient are different than that of the hypochondriacs. They do not mention all the symptoms or describe them in vague terms due to -  1. Indolence,  2. False modesty,  3. Mildness of disposition,  4. Weakness of mind. #Aphorism97 #Indolentpatients

Explanation of Aphorism 96 Hypochondriac patient.  Disposition of patient varies greatly, some patients who -  1. Are very sensitive and impatient of their suffering.  2. Express their symptoms in flowery colour, to induce physician to give prompt relief. They are hypochondriacs.  In footnote Dr. Hahnemann says that unadulterated and unexaggerated symptoms could rarely be received from hypochondriacs. It can be known by comparing their suffering complained of at various times and it subsides by giving nothing medicated.  From their exaggeration of symptoms, exaggeration itself becomes an important feature in constituting portrait of disease-case is exactly different with insane and malingers. #Aphorism96

Aphorism 95 Explanation Lesser accessory symptoms.  Lesser accessory symptoms are those which are often characteristic of the case but patient overlooks it because -  1. Patient is used to his long suffering, thinking it to be part of his daily life.  2. While narrating his sufferings, he forgets to mention it as he feels it to be normal, being suffering from it for decades.  3. He feels that these deviations have no connection with his principal malady.  But, these are important symptoms as -  1. These symptoms are often meaningful and characteristic.  2. Explanation of Aphorism 94  Finding obstacle to recovery in daily circumstances.  In chronic diseases physician needs to find obstacle from daily circumstances of the patient which maintains his disease and on removal of which his recovery may be prompted. Explanation of Aphorism 92 Conjoint malady.  In case of chronic diseases patient can be left with some unmedicated substance for some time, so that his actual symptoms appear enabling physician to form true portrait of the disease. But in acute diseases due to its severity and fear of eminent death, he cannot adopt the very process he adopts in chronic diseases rather he takes into account conjoint malady formed by features of original disease and medicinal action to form portrait of disease i.e. he only sees the present condition and prescribes for its totality.  If patient is given inappropriate drugs then condition becomes more serious and dangerous than the original disease, thereby requiring prompt relief; which end can be reached by taking into account conjoined malady, saving the patient to fall sacrifice to the injurious effect of drugs he has taken. 

Aphorism 91

Explanation of Aphorism 91 Symptoms appearing during course of medicine do not make true totality.  When patient is under course of medicine and some symptoms or altered feeling appears, it does not furnish true picture of disease as during that time the organism is under disease along with medicinal power. Thereby, symptoms appearing are due to collective action of disease and medicine going on within the organism - hence it is difficult to know which symptom is due to disease and which one is due to medicine.  Symptoms which were present before the use of medicines or has appeared after discontinuation of medicine for several days (when it is thought that medicinal action has been exhausted and the patient is apparently in his original diseased condition), are the ones which gives true fundamental idea of original disease which physician has to take note of.  In case of chronic diseases, when patient has been taking medicine up to the time he has seen the ph...

Explained of Aphorism 89 Need of asking special questions.  When the patient has finished on his own accord his story of suffering and has answered to the questions of physician, but physician thinks that he had not made complete picture of disease then he is at liberty to ask some special questions.  Physician has to rely on patient for description of his sensation of suffering unless it is a case of malingering. Even feigned diseases can be diagnosed by close observation.  In footnote Dr. Hahnemann gives example of special questions, avoiding its frame to be suggestive in itself, like -  •How often are his bowels moved? What is the exact character of the stool? Did the whitish evacuation consist of mucus or faeces? Had he or had he not pains during the evacuation? Explanation of Aphorism 88 Details lacking in voluntary narration can be asked.  In voluntary narration of the patient, if nothing important is mentioned regarding various parts or functions of body or mind, then physician has to inquire regarding the lacking detail. But, he should frame his questions in such a way that patient has to explain it in detail avoiding tag questions as already mentioned.  ► In footnote Dr. Hahnemann gives examples of framing of questions like -  1. Explanation of Aphorism 87 Physician should not ask tag questions.  After obtaining more precise information respecting each detail, without asking tag questions so that patient may not answer in affirmative or negative statement. He should not ask any suggestive question which has answer in itself as this answer could be misleading, either from indolence or in order to please his interrogator, from which a false picture of disease is formed resulting in unsuitable mode of treatment basing on this picture.  In footnote Dr. Hahnemann says that physician should never be guilty of asking suggestive questions, which may seduce the patient into giving a false answer and a false account of his symptoms.

Explanation of Aphorism 86 What to ask after patient had described his suffering.  When the patient along with attendant have ceased to narrate the suffering, physician reads the whole record carefully as if it was related to him and tries to elicit more precise and finer symptoms; examples of questions to be asked is mentioned in the text like-  1.At what period did this symptom occur?  2.Was it previous to taking the medicine that he had been using? .  3.While taking the medicine? Or only some days after leaving off the medicine? 4. What kind of pain, what sensation exactly, was it that occurred on this spot? 5. Where was the precise spot? 6. Did the pain occur in fits and by itself, at various times? Or was it continued, without intermission? How long did it last? Explanation of Aphorism 84 Procedure to be adapted in writing.  While the patient narrates his sufferings the physician begins to write every new circumstance with a fresh line and leaves a few lines before beginning a new symptom, so that he could add to any line the matter which had not been explained properly.  Some physicians while taking up the case writes rubrics instead of whole symptoms as instructed in this section, saving their time. But whenever that record is seen much is lost as the exact expression and words of patient is missing and we find only interpretation of the physician in the form of rubrics - which if goes wrong is tough to rectify due to absence of full symptoms.

Explanation of Aphorism 84 Primary instructions for history taking.  When the patient details his history, following are to be taken into account -  1. At the onset physician advises patient to speak slowly, so as to enable him to write down the important points what he says.  2. Physician maintains silence and allows patient and attendants to say regarding his suffering without interrupting him unless he changes his track.  3. Physician writes detail history of the patient which comprises of - a) Feelings and expression of the patient in his own words.  b) Attendant tells what they had heard him complaining of, how he behaved and what they have noticed in him.  c) Physician sees, hears, and remarks by his other senses- what is there of an altered or peculiar about him.  ► In footnote Dr. Hahnemann mentions the reason of not disturbing the patient when he narrates his suffering unless he changes his track as every interruption breaks his th...

Explanation of Aphorism 83 General directions for individualization of case.  Saying about individualization, Dr. Hahnemann gives general directions for individualizing a case but he says that every case is unique and requires different parameters which cannot be categorized to encompass each case as a rule. Rather, what physician needs to be present in him to handle every case before him is - 1. Freedom from prejudice, else he will be baised and his observation will be fallacious. 2. Sound senses, to perceive slight change in state of health. 3. Attention in observing, not to miss any point.  4. Fidelity in tracing picture of disease, to be exactly at the point.  ▲ Each case is unique and every case cannot be taken up using same protocol. Explanation of Aphorism 82 Individualization is necessary for cure of acute as well as chronic diseases.  Discovery of chronic diseases along with their specific homoeopathic remedies have advanced much in attaining cure; yet individualization is must. Only difference occurs in acute cases when it develops so rapidly that the chief symptoms becomes self-evident to senses more quickly resulting in to trace the picture of disease within no time; contrary to chronic diseases which gradually progresses for several years, resulting in symptoms to become more difficult to be ascertained.  In footnote Dr. Hahnemann says that directions regarding self-evidentity of symptoms are only applicable in acute diseases. #Aphorism82

Reasons for such increased number of secondary symptoms of Psora are   1. Different corporeal constitution of people had been affected by it. So, it had gained varied forms.  2. Even today's medical science believes it to be hereditary.  But, Sycosis was not recognized as chronic miasmatic disease; although -  1. It cannot be eradicated by vital force alone unless cured by proper anti-miasmatic treatment.  2. Explanation of Aphorism 77 Introducing inappropriately named chronic disease.  In this section inappropriately named chronic diseases, having following characteristics are mentioned:  1. Occurs when person is continuously exposed to avoidable noxious agents-  a) Habit of indulging in liquor,  b) Addiction or dissipation of many kind,  c) Prolonged abstinence from things that are necessary for support of life,  d) Residing in unhealthy locality especially marshy district, who are housed in cellar or other confined dwellings,  e) Deprived of exercise or of open air,  f) Overexertion of body or mind,  g) Who live in constant state of worry etc.  2. Under proper diet and regimen along with healthy mode of living these states of ill health disappear spontaneously, provided no chronic miasm lurks behind in the body - so, they cannot be called as chronic diseases.

Aphorism 76

Explanation of Aphorism 76 Treatment of Iatrogenic disease.  Iatrogenic diseases are difficult to cure and homoeopathy is meant to cure only natural diseases, here Dr. Hahnemann says regarding treatment of iatrogenic diseases-  1. Initially the medicine which is thought to be the major cause of the condition is to be stopped, letting the vital force to remedy itself.  2. If vital force is unable to overcome it, due to presence of chronic miasm, then appropriate anti miasmatic treatment is to be instituted. 3. To carry out the above mentioned two points, it requires time of several years.
