Explanation of Aphorism 81
Psora is thousand headed monster.
 Psora being most ancient miasm had gradually passed many. generations through millions of human organism and attained such vast development which we call secondary symptoms of Psora, although it possesses a distinct name and is believed to be an individual entity of independent character by old school. Reasons for such increased number of secondary symptoms of Psora are 

 1. Different corporeal constitution of people had been affected by it. So, it had gained varied forms. 
2. Different constitution had been under varied external and internal factors which had added to its different forms. 

In first footnote Dr. Hahnemann mentions some of the modifying factors which helps in transformation of Psora into chronic diseases

 1. Climate, physical character of place of residing. 
2. Physical and mental training of youth-delayed, excess or abuse. 
3. Diet and regimen. 
4. Passion, manners, habits and customs of various kinds. 

In second footnote Dr. Hahnemann puts a question mark on nomenclature of diseases depending on group of distinct character and treating it accordingly as if they were of unchanged character. Although, the diseases they named resemble other of same name in its few characters. To show this type of nomenclature and same treatment basing upon its name illogical he refers to writings of

 - 1. Huxam, 
2. Frittze, 
3. Sydenham. 

From these attestations it is clear that it is only misuse of names of diseases to begin same treatment of patient laboring under same named disease rather a true physician is not influenced by it, as he knows that in order to cure he has to take into account totality of the case in hand. 

Nomenclature of diseases is unimportant in therapeutic point of view unless it is of fixed character save for conversa on, when it is better to use words like a kind of fever, a kind of dropsy etc.

Explanation of Aphorism 84 Procedure to be adapted in writing.  While the patient narrates his sufferings the physician begins to write every new circumstance with a fresh line and leaves a few lines before beginning a new symptom, so that he could add to any line the matter which had not been explained properly.  Some physicians while taking up the case writes rubrics instead of whole symptoms as instructed in this section, saving their time. But whenever that record is seen much is lost as the exact expression and words of patient is missing and we find only interpretation of the physician in the form of rubrics - which if goes wrong is tough to rectify due to absence of full symptoms.
